In this issue the microscope is shone above the transition of patients from the acute secondary care setting into primary care. "If not you, then who?" Steven Williams asks in a guest editorial. This is a reflection on work conducted by EAHP in collabortation with Euro Pharm Forum regarding the level of collaboration of pharmacists in different care settings, community and hospital pharmacy. Similiarly the issue of medicines reconciliation in 30 actue hospitals in the UK has been reported, patient centred clinical services using the Lund Model from Sweden and influencing prescribers with pharmacy led education in Denmark can all be found in this latest issue.
