Statement 5.7

“Hospital pharmacists should ensure that the medicines administration process is designed such that transcription steps between the original prescription and the medicines administration record are eliminated.

What does it mean for patients? Transcription (handwriting) of the medical prescription onto the health care record of patients is prone to errors and should be eliminated by implementing safer systems.

What does it mean for healthcare professionals? In order to achieve patient safety transcription of medical prescriptions on patients health care records should be eliminated as it is known to be a major cause of medication errors.

What does it mean for Hospital Pharmacists? Hospital pharmacists should define together with other healthcare professionals the medication process to proceed with prescribed medicines and their administration without transcription. Ideally an electronic integrated system of prescription, pharmaceutical validation and administration should be defined by users, implemented and monitored.
