ESCP International Workshop Improving Patient Care through Collaborative Practice

Patients and their families expect that health care providers will collaborate together and work as an efficient team in the delivery of safe and effective patient centred care. Patient experience, teamwork and outcomes improve when there is a shared understanding of the treatment plan for individual patients. Our ageing population and the increasing prevalence of chronic disease are drivers for interprofessional teamwork and collaborative practice. In fact, lack of effective teamwork and interprofessional communication is considered a cause of poor patient care and outcomes.

 

There are many examples of pharmacists working as a member of the multidisciplinary team in hospitals, out-patient clinics, primary care, nursing homes and community pharmacy to identify, prevent and resolve medication related problems and reduce inappropriate prescribing. Roles often include leadership in maintaining continuity of pharmaceutical care during movement between health care settings.  Examples of collaborative practice will be shared in this workshop to stimulate practice developments across Europe.
