Abstract
 
Sometimes, the occurrence of illness deeply affects the quality of life of patients, producing a feeling of disempowerment. Healthcare professionals who take care of them, particularly when they suffer from a chronic disease, should be always aware of this reality. 
However, even if it should be obvious that an effective therapy cannot be limited to the choice of “right medication”, today’s medicine is increasingly oriented to a “disease-centred” impersonal approach. 
Therapeutic Patient Education (TPE) is a “person-centred” therapeutic approach and procedure providing patients and/or their caregivers with the skills to self manage the disease and prevent complications TPE also offers a deeper understanding of the disease, helping patient empowerment.
In contrast, if healthcare professionals do not teach patients how to self manage their disease nor to provide them with necessary understanding, the risk of complications significantly increases. 
 
As a consequence, besides patients, TPE also may benefit their families, health care community, insurance companies and taxpayers.
 
For all these reasons, the pharmacist is also invited to get involved in TPE that should be seen as a regular part of standard Pharmaceutical Care. 
 
However, to perform TPE, every pharmacist should receive a specific training in order to optimize his professional practice by the implementation of TPE procedures.
 
This workshop will briefly discuss the systemic approach to patient education, going through the different steps of a TPE program, from educational diagnosis to result evaluation.
 
Teaching Goals:
 
  • to describe the systemic educational approach and TPE, 
  • to illustrate the different steps of a TPE program, 
  • to define the key role of pharmacists in TPE.
Learning Objectives:
 
After participating in the workshop the participant should be able:
  • to describe methods/tools that can be used in a TPE intervention, 
  • to identify patient needs,
  • to explain the importance of the partnership with the patient and his/her caregivers,
  • to set up outcomes within a TPE intervention,
  • to assess patient’s learning/understanding.
