Learning Objectives:

At the end of this session, the participants should:

  • Understand the expanding pharmacists’ scope of practice to improve patient care and access to affordable healthcare
  • Know how innovative digital tools such as Bayer’s Smart Care Cases, Smart Avatars or Smart Cards facilitate continuous pharmaceutical education and improve patient consultation in daily practice
  • Know how to address common implementation barriers and rethink the need of digitization

Educational need addressed:
Hospital pharmacists need to ensure excellent pharmaceutical care in their patient consultations, particularly providing tailored information on disease and anticoagulation management to each patient – despite time limitations. Information on digital and interactive training and patient support tools is provided including a demonstration of Smart Cards that can facilitate patient consultations during daily practice. In addition, practice experience is shared how Smart Cards can also support patients alone at home and can help them to improve their health literacy as well as their disease and anticoagulation management skills across diverse cardiovascular indications.
